Registering for Expecting and Connecting Programs

Who is eligible? 

  • Pregnant people who are NOT already receiving care from a Registered Midwife

  • Pregnant people and their partners living in Windsor-Essex County

  • People have their first baby or people who have already had children

  • While midwives take care of healthy, low-risk women, you can still see a midwife, even if you are not having a low-risk pregnancy 

Group Prenatal Care

Please fill out our self-referral intake form OR have your family doctor or nurse practioner fax a referral to our office. 

Please note: This program supplements the prenatal care you are receiving from a doctor

Group Postpartum Care

Please fill out our self-referral intake form OR have your family doctor or nurse practioner fax a referral to our office. 

Please note: This program provides most of the postpartum care you and your newborn need in the first six weeks have the birth. You would only need to see another healthcare provider if you need help beyond the scope of the midwife. 

Postpartum Home Visits

Please fill out our self-referral intake form OR have your family doctor or nurse practioner fax a referral to our office.

What happens next?

 

Once our office receives the referral (from you or your doctor), your application will be evaluated. 

This may include our office administrator emailing or calling you to ask you additional questions.

If there is space in the program and you are a good fit for Expecting and Connecting the administrator will provide you or your doctor with the date and time of your first appointment.
